Patient Financial Responsibilities

 Before receiving care at Ramin Mazhari LLC, all patients must complete a registration packet and sign our formal Financial Policy. This agreement establishes that you are responsible for understanding your specific insurance benefits, including network status, referral requirements, and prior authorizations. To maintain your account in good standing, you must present a Photo ID and insurance card at every visit and provide payment for all copays, known deductibles, coinsurance, and past-due balances at the time of check-in.


While we submit claims to your insurer, the insurance company makes the final determination of eligibility and coverage. You are responsible for any portion of the service not covered by your plan, and all remaining balances must be paid in full unless otherwise agreed upon with the Practice Manager. All outstanding balances must be resolved prior to scheduling or attending future appointments.

At Ramin Mazhari LLC, we believe that a successful physician-patient relationship is built on a foundation of clear communication and mutual respect. To ensure we can dedicate our full attention to your clinical care, we ask that you review and follow our financial guidelines:

  • Arrival Essentials: To protect your identity and ensure accurate billing, please present a Photo ID and your current insurance card at every visit.
  • Payments & Balances: All co-payments and any outstanding balances are due at the time of check-in. 
  • Accepted Payment Methods: We accept cash, credit cards, and check.
  • Keep Us Updated: It is your responsibility to notify us immediately of any changes to your contact information (address/name) or insurance coverage.


If you have any questions regarding our fees or your specific responsibilities, our team is here to help. We truly appreciate your cooperation in helping us maintain an efficient and patient-focused practice.

Self-Pay

Patients without active insurance and those whose plans we do not participate in are classified as self-pay accounts. We offer transparent pricing prior to your visit so you can plan accordingly. Any out-of-pocket services rendered during your visit must be collected before you leave the office. For those wishing to seek potential reimbursement from their insurance, we can provide an itemized superbill for you to submit your own claim.  It is the patient's responsibility to verify our participation in their specific plan prior to the visit. In the event of an insurance discrepancy, accounts will be treated as self-pay until coverage is confirmed. Because our primary goal is to provide exceptional care without causing financial hardship, we offer payment arrangements.
